Double post

I have this issue on a client WordPress site where it is double-posting an image. I can text from my iPhone, and it will post one photo. But when my client posts from his phone, which I think is a Samsung Android, it posts the same image twice in the post.

The image template currently is:
<img src=”{MEDIUM}” alt=”{CAPTION}” width=”{MEDIUMWIDTH}” height=”{MEDIUMHEIGHT}” class=”alignnone size-medium wp-image-{ID}” />

We just updated to 1.6.1 today, so it was last tried with the previous release.

Any ideas?
