It seems that you are having some conflict on your site since WooCommerce will not automatically refund twice when it is intended to refund shipping costs only once.
The refund ID is also not the same, so it doesn’t seem to be a duplicate. Can you please check the Order notes logs for further details on these refunds?
I’d suggest monitoring Github for any other user reporting the same issue, even if it currently seems isolated to your environment: https://github.com/woocommerce/woocommerce/issues.
You could do tests with PayPal Sandbox and see if the problem recurs. For conflicts, you can find a more detailed explanation here: https://docs.woocommerce.com/document/how-to-test-for-conflicts/.

@rainfallnixfig I’ve just had a client have the same bug, where the super admin (me) was showing as a duplicate refund exactly like the attached screenshot. Paypal wasn’t putting through two refunds its more visual but still concerning 🙂

Same problem here. Super-Admin does an automatic second refund shortly after refund created from normal user.
I think it has to do with installed “WooCommerce DHL Plugin”.
