Duplicate entry fields

Hi, i installed pirate forms two days ago. everything was working good and smoothly. today i noticed all the new widgets i create and update end up have duplicate / double information entry fields above where the message is made. I already tried deactivating /reactivating. then i deleted it and reinstalled it. same problem. I like the plug in but i cant be confusing our clients so the sooner i get this resolved the better!
