Duplicate warning box

Hi, I have an issue, hope anyone can help my with this:

When a contact field is missing or the user hasn’t answered to a required question, a warning box appears, but duplicated. Also, it says that such field is required, but without any further detail about which one.

I don’t know if this is a PRO feature, there can be some issue with my theme (LMS)…
