This is caused by rewrite rules not working on your site. There are two things to check:
1). Go to Settings, Permalinks. Visiting this page will flush your rewrite rules.
2). Check your htaccess file on your server to make sure it looks ok. Here’s the documentation on this – http://codex.ww.wp.xz.cn/htaccess
If neither of these work then it could be a plugin conflict.
1) Have refreshed many times, no change
2) HTACCESS seems to be fine, everything is default
I am operating with minimal plugins and have tried deactivating, still not working…
This could be your theme that is causing this problem. We’ve seen this before where plugins or themes flush the permalinks on page load which is a bad idea.

The way to troubleshoot a theme or plugin issue is to deactivate all plugins except All in One SEO Pack and then switch themes to Twenty Fourteen or Twenty Fifteen. If everything works fine when running All in One SEO Pack on one of those default themes then start activating plugins one at a time until the problem reoccurs. If the problem reoccurs when you activate a specific plugin then you’ve found the conflicting plugin. If it does not then you’ve proved it’s your theme.
