Easy Appointment error with Modal

Hi, I’m using Avada theme, and when I add the plugin shortcode inside the Modal, it automatically closes the modal itself.
Another question is: where do I download the Easy appointment connect plugin?
I wanted to test the google calendar connection and paypal connection of this nice plugin of yours.
The last issue I have is that for some reason the selection of the service, location, and worker is not appearing ( it did the first time but is not appearing anymore, and i cleared the browser cache but still the same) suggestion? many thanks!!
