Edit doesn’t work

So I cannot actually edit any custom provider. When I click on “edit” which opens pop up window, in that window provider name and country are not prefilled. I mean if I am editing a record then it should prefilled default values instead of showing blank text boxes.
Then when I pressed “Update” button, it removed provider name and country. Now when I am trying to edit again and even after entering provider name and country, it doesn’t save and just shows red boxes around provider name and country indicating there is some error but doesn’t show any error message.
What a shit show!
