Looks like a broken stylesheet to me. Empty all your caches and reload.
If it persists, check for errors in your browser console.

Also tried opening the console to see if i can get any error to pop up but its empty, could you please provide more details regarding the troubleshooting?

If the console is empty then I guess there are no errors.
I can’t explain how this can happen if your cache is empty and there are no errors loading resources.
Every new release of Loco Translate will be different, so a broken install that doesn’t update all the files could cause this. Also an aggressive cache that won’t load new resources could cause this.
My only remaining suggestion is to delete and re-install the plugin (to ensure all files are present and correct) and if that doesn’t fix it, disable ALL OTHER plugins (to ensure against any conflicts).
