Sorry, I don’t see the Webba Booking form on this page (only contact form). To add the Webba booking form on a page, please use the shortcode [webba_booking] .
By the way, please clarify – are you using lite or premium version?

Have you added [webba_booking] shortcode on your page? You don’t need to use shortcode from the Contact form 7.
Yes, I have added. [webba_booking] short code that’s why services selection is coming… the Zip Code is different services I have added
I need different fields than your default form, that’s why I used webbooking form short code in contact form 7
On page http://www.simlaus.com/schedule-pickup/ I can see only contact form, not a booking form. Can you send a screenshot of shortcode usage to [email protected], please?
