If you could send us log file (to [email protected] ), you will find it in plugins directory:
../revision-manager-tmc/.log/
it may help us to see what is going on!

Hi @ecmatodot! We have not received an email with log file and info about your environment from you yet.
Just for your information, tomorrow we will update plugin. You will see new option in the bookmark “Tools” > “System info”.
I want still our help, after you make plugin update I will ask you to send us “System info”, so that will help us to check your environment and fix bug if it exists.

Thx for feedback & contribution. I think we may close this topic, the problem was solved.
Just to let know what was wrong:
1) “send test” button is grayed out. (solved) – the problem was with the loading of scripts, eg regarding the appearance of the plugin in the windows server environment. Next update of Revision Manager TMC fixes the problem.
2) “Email notifications to the Administrator are not working” (solved). That was not a bug. Problem was the settings of the server. PHP function to send mails did not send e-mails. Solution is to install free plugin to set SMTP allows you to configure a mail server which handles all outgoing email from your website. e.g. SMTP Mailer
