Looking at the screenshot on https://ibb.co/TcVZt64, it seems the Email Summary is from WP Mail SMTP, and not WPForms. But looking at the second screenshot on https://ibb.co/VBh0psK, it seems you disabled the Email Summaries on WPForms.

To disable the Email Summaries on WP Mail SMTP, please follow this guide.
