Erro ao criar integração

Olá pessoal, esperto que esteja tudo bem com vocês.

Instalei o plugin para integrar com o Gravity Forms mas ao criar a integração aparece o erro abaixo e não aparece a seção para mapear os campos dos formulário.

Se puderem me ajudar, agradeço.
