    Odd. Please send a list of active plugins and the Nirvana options export file to me at chris at wpmission.com. I will try to re-create the problem.

    This was solved by deleting and reinstalling the plugin. (Testimonials and settings are not deleted.)

    I have this same issue with my Strong Testimonials form on my website – http://www.laststopcounsellingservices.co.uk/site/testimonials/

    I have tried the following 2 possible solutions without success:
    – I have deleted and re-installed the plugin
    – I have deactivated all other plugins

    salakoao found out that having a field named “name” was the cause. Using the default “client_name” or something other more just “name” fixed it.
