What does your error_log say?
Since you’re running Windows, I really am not sure. Google sounds like our friend now.
Did you have this plugin installed and working before?

You might consider setting up a dev site in which you deactivate all plugins and change to theme twentytwelve first. Then install TW, then try adding in the other plugins or changing themes to find the real conflicts.
I followed the debug steps and all was good. I also updated to WordPress 3.7 and all was good. I am thinking maybe there was a fix in 3.7 that was affecting my theme/plugin mix. Thanks for the help!
