Error database error

    Hello!

    I get this error when i activate the plugin

    WordPress database error: [Table ‘devdigit_srw.sr_sib_model_forms’ doesn’t exist]
    DESC sr_sib_model_forms
    Any way for me, to fix it?

    +1 same issue with version 3.0.4

    I tried installing the plugin today and it doesn’t work due to the exact same issue.

    Same issue:

    WordPress database error: [Table ‘dbwcnubyns67t5.wp_sib_model_forms’ doesn’t exist]
    DESC wp_sib_model_forms
