1. what version of wp and das are you using?
2. what theme are you using?
3. have you tried disabling all your other plugins and try to activate again?
4. have you tried a different theme?
Wp 3.5.1 – just upgrading now to .2
Theme – Attitude

Still the same problem. I am getting following error…
Fatal error: Cannot redeclare st4_columns_head() in /home/libram1m/public_html/xxx-domain-xxx/wp-content/plugins/design-approval-system/includes/das-functions.php on line 33
Hope this help you resolve it.
You know what that was a new implementation that did not get completed. I will be commenting out that code and making an update now, that should solve the problem! Let us know if that fixes the problem.
