Error in Form Validation Rules

I am using a Mailchimp newsletter sign-up form on my website: http://www.stempilot.com (bottom of page). When I submit an accurate/valid email address and all required fields, I get the following message: “Please provide a valid email address.Please fill in the required fields.” — instead, the success message should be displayed. I am also not receiving an email letting me know that someone has successfully signed up. Can you help? Thanks!

I think the problem is that the name of the email field is changed to name=”Join Our Mailing List”.
Please delete the field in the form editor and add it again so the correct name will be set for you.

Additionally, I have been receiving emails from WordPress alerting me to new user registrations on the site — the emails include the following (I removed the actual username/email here to protect privacy):
New user registration on your site STEMPilot:
Username: XXX
Email: XXXXAre these registrations coming from the MailChimp form? There is no field for ‘Username’ in the MailChimp form, but I am not sure where else they would be generated from.

These emails are sent when new user sign-up on your website so it would not be our plugin that is generating those emails.
That is odd, as I have no other forms on the website that include a ‘Username’ field, and the emails are inconsistent with the alerts.
Is there a way to set up an email notification when someone signs up via the MailChimp form?

Please do check your “Users” page in WordPress to avoid any security issue of users signing up on your site.
