    Hi!
    Right after the activation of your (very beautiful) plugin I see this error message:
    “wpcf7_add_shortcode is deprecated since Contact Form 7 version 4.6! Use wpcf7_add_form_tag instead.”

    I’m using Contact Form 7 Version 5.0.4
    Maybe changing some lines in your code will do the trick.
