    There is definitely something odd going on. The jQuery script which Google Forms uses appears twice in the HTML source which is wrong. It almost looks like wp_head() is being called twice.

    Do you have more than one form on the page? It sort of looks that way.

    Hi Mike,

    That was true, I added your demo link (which worked) just to see where the problem lay. Now I cleared everything and added the shortcode in the html part, and it works again!

    Don’t know why since I tried that before as well, but nevermind.
