Theme:Twenty Twelve
We tested the plugins on multiple sites with WP3.9.2 and theme Twenty Twenty and we were still unable to re-create the issue.
Although we weren’t able to re-create the issue, we just published a new version of the plugin which might address your issue. Please try version 1.0.5.

We were able to re-create and fix the issue. Zendy Mailman 1.0.6 fixes the activation crash.
