Error when importing sample data

When i try to import my data, i get an error screen.

when im on the import screen there is code showing at the top of the screen also. (see image at link http://atlfire-sds.com/wp-content/uploads/2013/09/Screen-Shot-2013-09-25-at-1.00.42-PM.png ) i cant figure out the issue, ive reinstalled my files and still no go.

need to get this thing up and running any help is greatly appreciated. thanks.
