Error when submitting form – required fields

I have created a form within my blog for clients to submit a questionnaire. Most fields are required. I asked a friend to test it and she said when she submits her form it prompts her that the Reservation field is a required field (Reservation is the first required field I have). I tried changing it to not required and it just went to the next required field. What’s going on? I need this sent out today!
