This is strange. There was an issue with submission of the form in one of the previous version of the plugin but it was resolved: https://ww.wp.xz.cn/support/topic/review-not-appearing-on-product-page/.

Can you please go to WordPress Settings -> Permalinks and tell me what option do you have in Common Settings?
Even better if you can make a screenshot showing your current settings on that screen, upload it somewhere (e.g., Dropbox) and share the public URL of the screenshot.
Thanks for providing the additional information. The problem is that your blog doesn’t have pretty permalinks enabled.
I’ve just released version 3.5 of the plugin. It should work with any configuration of permalinks (including yours). Please install the new version and send a new review reminder email to test the review forms.
The old forms generated by the previous versions of the plugin will not work even after the update. So, you need to send a new review reminder email (either for one of the existing orders or for a new order). You can easily do it by using the function for manual sending of review reminders.

Same thing here, created an order and received the mail acting as a normal customer. I hit the “submit” button and then I get a message saying “Error while submitting form”.
@antotonydc, it looks like REST API is disabled on your WordPress website. You should enable it to let the plugin receive customer reviews.

WordPress REST API is still disabled. The server hosting reviews forms cannot send them back to your website because your website rejects REST API calls.
