Event Actions all show same count

We have a multi step form which there’s no way gets 100% completion for every step of the form for every submission, but this is what is being reported on analytics.

I thought that maybe all actions/labels were being loaded on every step of the way and that this may be why everything is reporting back to google analytics as specified above, so I built in some conditionality as shown here, but still no joy.

I’m sure that this is user error of some sort, but can’t figure it out and hope that I can get some assistance so we can start seeing proper representation of funnel completion in analytics.
