Event Registration Developer Still Alive? Layout Problem! Hellooooo!!!?

Event registration will not properly layout its form. If you go to this form:

You will see at the bottom of the form that the popup selections do not appear in the right place. In the source code, the input field comes BEFORE the label field. I cannot find the place in the code where I can fix this. I’ve had to use very kludgy workarounds for a similar layout problem AFTER posting questions on this board and via email to the developer. No response.

If you’re going to put this out there, then please support your work.

Can anyone recommend a fix, please??
