Is there any site/server wide caching going on?
Do you have any other WooCommerce related plugins that are adding filters to ‘woocommerce_get_availability_text’?

Just realized that I deleted most of my title, sorry about that!
No server wide caching going on. No caching plugins.
I tried deactivating all plugins apart from WooCommerce and Cart Stock Reducer, with no luck either.
Another thing to note is that ‘Append Included Items to Pending Order Text’ works fine.
Is there anything unique about the items that it is happening for vs not? Simple vs variable products?
Are there perhaps items in carts from when the plugin wasn’t active?

No, there wasn’t anything unique.
I didn’t think so but I decided to clear the current woocommerce customer sessions under woocommerce > status > tools just in case. While there I found buttons to clear ‘WooCommerce transients’ and ‘Expired transients’. Seems like one of the few things I did in that panel did the trick, unfortunately I don’t know what it was for sure.

@jamesgol An update: The problem started to occur again so I decided to go through the above one by one to find out what was causing it. Clearing the customer sessions is what solved the issue.
Is there anything you can think of which may solve this issue? Clearing customer sessions works but is obviously not an ideal solution.
Any help is once again appreciated!

Can you create a database dump when this happens and send it to me? I’d be curious to see what is in the WooCommerce session tables that isn’t getting expired out.

Sure thing, if/when it happens again, I’ll do that. How exactly do I create the database dump you’re needing?
I’m not sure what your setup is like. If you have command line access you can use mysqldump, if you have phpmyadmin you can do it through there, there’s also lots of WordPress plugins that can create a database backup.

I’ve also noticed something strange. Last night I checked my site, around 40 items were on hold. Weird. So I decided to link up google analytics. While doing this, another 20 items were reserved. Then again at around 2-3am this morning another batch of items were reserved. Analytics doesn’t have any record of sessions outside of the ones I started to check that the numbers were correct. Could it be a related internal error?
I haven’t looked at the file yet, but you should send this stuff privately. I don’t have any way to delete or edit your message but you probably should remove it in case there is any user information or other sensitive data.

After troubleshooting the issue was fixed in version 3.05
