Extra “Add item” form element created

Hi, Just starting using this plugin and so far it suits my needs. I created a simple form with a field group encapsulating a set of radio buttons and a text area. Below those are a phantom element called add item. I didn’t explicitly create this and dont want it on my form. I figured it was there if logged in but not when logged out, but that is not the case. It is always showing. When clicked, it duplicates the entire form and then adds a remove item button. Is there a way to remove it?
