Failed to Geocode

I previously was able to enter new locations without a problem. I only have two test locations in right now, but I keep getting the following:

Total attempts 1, waited up to seconds between request.
test Added Successfully.The locations is not added. I have not changed ANY settings at all since the last time I added a new location. I don’t understand the OVER_QUERY_LIMIT since I have really only tried to enter a few locations.
