Hi – I’ve tried this is a few times using the setup as you (I assume you mean the latest version of QTL) and can’t reproduce this. I assumed it might be a Gutenberg issue, but even with GB on and classic editor on I get no error messages 🙁
Is it on a particular page?

I’ve spent two hours this weekend trying to reproduce but have had no luck.
Can you confirm the images are in the reverent folder.
Are there any javascript errors in the console?

I solved.
The plugin folder was quiz-tool-lite-2.5.0 but the plugin looked for the quiz-tool-lite folder. I changed the name to the folder and now it works.
I do not understand why it stopped working suddenly.
