Feature Image Problems

I’m having problems with the feature image function. The majority of images I set as featured appear in posts perfectly. Others are cropped or pixelated. There seems to be no rhyme or reason as to which images this happens to.

Is there a standard image size I should be using to ensure consistency of image display?

I am running the Twenty Ten theme functions.php with a custom-built theme.
