feature suggestions / requests

Hi!

First, thanks for the great plug-in, it works great. Two feature requests / feature suggestions:

1. Set a second COD cost tier

Some of our delivery partners have a tiered cost structure for COD extra fees:
1. a fixed fee below below a certain COD transaction value,
2. a % based fee (replacing the fixed fee) above a certain COD transaction value

It would be great to have an option to specify a second COD fee with a condition (“if transaction value is below: …” / (“if transaction value is above: …”, and the plug-in would charge either the default value, or the other fee if condition is matched.

2. Different maximum COD value definition for different shipping methods

As said bfore, we work with different shipping companies and they have different maximum transation values defined for COD money collection. Accordingly, it would be great to be able to enter maximum transation value for COD by shipping method.
