I see the thumbnail attachment copied from 1409 on the parent post to 1386 on the child post.

So I see nothing wrong here.
Is attachment 1386 on the child blog the correct image?

The file should exist on disk for both blogs.
Perhaps one of the many, many plugins you have active is a problem? Perhaps WP Smush, 2.4.3?
My suggestion is to start disbling plugins, one at a time, to see if any of them is causing a conflict. According to the debug dump, the image exists and was correctly copied.
the files are there. finally figured out the offending plugin: Post Type Switcher. i’ll mark this as resolved already.
