Featured Image not working

So I’ve got my form all set up: http://www.truthaboutdiamonds.com/upload-your-ring/

The “ring photo” field is supposed to create a featured image from the attached image. The upload works. The file is in the GF uploads directory. The image shows up in the notification email.

But in the newly created post, I get nothing. Just the text from the “your story” field. No featured image.

Yes, I have the “use as featured image” checkbox checked.

Any ideas what could be the problem?
