Feedback button and form

Hi again,

So I’ve ran into what seems to be my last troubleshooting problem I should ever run into again. If you go take a look at my blog (http://blog.groupideo.com) you see I have a feedback button on the left side of my blog that when clicked opens a feedback form for someone to fill out their content. Now if you go to any other page on my blog for example (http://blog.groupideo.com/what-is-social-video/) and click on the feedback button on the left side it doesn’t bring you to the same form. In order for me to have gotten my feedback form to work on the root of the blog I had to upload my files to a root folder.

I guess my question is why the heck wont it work on my other pages and where should I upload my files in order to get it to work. I’m sure their are some admins and creators and lots of skilled people using WordPress that will be able to answer my question.
