There is no contact form built into Twenty Seventeen so it must be coming from a plugin. Check which plugin it is and try looking at its documentation or posting in their forum. If you aren’t sure what you’re running, can you provide a link to the page with the contact form on your site?

That’s odd. The theme came with constant contact plugin pre-installed. I removed it, but the contact form still appears.
