Hi,
i would like to know if there’s a way to add a SURNAME field before NAME.

vorrei sapere se c’è un modo per aggiungere un campo COGNOME prima del NOME.

Hi, it’s not possible because name and email are 1st and 2nd fields, THEN you can add custom fields, as shown in this tutorial:

Then, you could write some jquery code to change the field order.
