Fields clear, upload fails

I have about 10 different upload links and they only seem to be working for me, and even then only sometimes. Once a user hits “upload file” there is a pause, the fields clear, and it fails to reroute to the success page. I added a message to try and diagnose the issue and it said “file not allowed” but I didn’t set any restrictions on that upload. My first file upload requires a PDF (I have 4 nearly identical instances):

I have another four that can accept any file type but for some reason users cannot upload an .mp3, which is the file type that gave me the “file not allowed” message. I am having about a 3% success rate with outside users and I don’t know why!!
