Any more details about this file, is it a core or plugin/theme file? also, when you run a scan do you get this file listed as being modified there?
In general, I suggest making sure this file isn’t listed in “Comma-separated list of directories to exclude from recently modified file list” option in (Wordfence > Options => Email Summary).
