    The one at the bottom corner? The form comes up (I did not fill it, ask a friend of yours to fill it from a different computer).

    Yes, bottom right widget. We have tried with other PCs with same negative result URL NOT FOUND on this server and yet another blog running the same widget is perfect.

    Yes. On filling up the form, I too get the ERROR message. Try reinstalling it, or use another plugin.
