The forms seem to be appearing and working just fine. I had refreshed the page several times and the forms still are there.
the form is appearing because I made another clone of it and used it. two times the form just stopped and disappeared from my visual composer. Forms just stop working with no changes being made to the site, here are links to the two forms that do no work anymore. You can see the shortcode but no forms.

Upon looking at the links you have provided me, there is an Error 404 which simply means the page does not exist. If the page does not exist, then the forms of course will not show up.

I would suggest you to use these shortcodes:

gone again, it continually just randomly disappears.
im putting form in visual composer, it works then randomly I get a call saying its gone. If its not compatible I need to know so I can find another solution but it keeps disappearing and that cant happen. any solution would be great.

It sounds like Visual Composer is stripping the shortcode for some reason. All things being equal it works so it has to be something funny with Visual Composer. I wish I could say this isn’t common, but with every update to Visual Composer we get someone who says that it’s borked something.
