Form Actions

Hey there,
Having trouble trying to get my form to use the action of its own page. I think the problem stems from dynamically created pages and not being able to target them correctly.

Basically I have a form to email functions build into the PHP at the top of the page, and I want to use the action of the page the form is on to fire that PHP. When I send my form it just reroutes to the index page and the form does not send to email.

If anyone has any tips that would be awesome
