Form changes – Passionate theme

Hello I am seeking some guidance in changing the dates on a form on my website. Here is the location i am working on in my site: http://hylaumc.com/special-dates-deadlines/online-application/. Once the user has selected the form and location the dates show up but they are not updated. I am wondering how I can change the dates? I have gone into the ‘Form Visual Builder Pro’ but I do not see a simple way to change the dates.. Please help. Thank you
