Form Data Picker is Corrupted

    Sometime in the past few months, the date picker form field has stopped working properly. We are using the ChurchAdore them and Form Maker Pro 2.12.13

    1. The calendar modal overlay is corrupted (it’s semi-transparent and is difficult to read).
    2. The dates that are rendered in the input box are invalid. They show up as %2-%8-%Y. It’s like the stylesheet simply isn’t working.
    3. I cannot edit the date field in the form editor. When I edit the field, the entire page is blank except for the update button.

    This problem does not occur with WordPress 4.8.5 with the Form Maker Pro 2.11.11

    FYI: This turned out to be a defect in the form maker plug-in. The version that resolved the problem was:
    Version 2.12.17
