Form date created: Invalid date

I have two forms activate. One of them shows an “Invalid date” entry as the date created in the dashboard. Any new forms created show the created date just fine. It’s just one form that has this issue. When I duplicate the problematic form, it shows the created date fine but when I refresh the page, the invalid date message appears again for that form!
