Form disapear

Hi,

first,
PHP Version: 7.2.14
Wordpress Version: 5.2.2
Plugin Version: 5.9
Is Pro User: YES
Current Theme: Suffice

I think there is an issue with the form. When i reduce size of main page (the page where the form is availaible) a button appeared and the form is located just below.
If I click on this button, the form appear and disappear, OK.
Now if I clicked to not diplaying the form, and I re-full size the page, the form is lost.
To get it again, i need to reduce the size, click and resize.
