it is pasted into a site origin text editor (from site origin widgets bundle). It was working for months but it has disappeared. I don’t know when. Maybe something is clashing with it ?
I recently installed plugin Product Enquiry for WooCommerce by wisdmlabs. This has a small pop up form. Maybe this is blocking the form ? I have no idea. I am out of my depth
I also tried to create a new form and i fill out all the labels but can’t save it

I’d agree that there could be something clashing — (theme functions can interfere with plugins, as can other plugins)
To find out, you could try deactivating all plugins. If that resolves the issue, reactivate each one individually to see if one had contributed to the symptom.
Additionally (or instead), try switching to the Twenty Fifteen theme to rule-out a theme-specific issue.
