All looks great on PC but on an iPhone in portrait orientation I cannot see the form entry field on the right, only the name of the fields.

You can try using the bootstrap template…

That work indeed, but where can I edit the styling ?

The plugin doesn’t have an interface for editing template styles. It relies on your WP theme to do that. If you need to add special CSS rules for the plugin displays, there is a place to put your rules in the Custom CSS setting in the plugin settings.
