Form Does not fill out

I have WP 3.5.1 and ER to 6.00.31

When doing a test run, I am unable to type into the registration form and have it show what I am typing. Part of the correct info show on the next page. When filling out the form, I indicated the number of tickets I want to purchase (2), and the number of attendees shows as “0”.

I get the following 2 messages/errors:
You are registering for 0 person(s). Please provide the first and last name of each person:

You must select at least one registration item. Please go back and select an item!

Any idea how to correct this?
