Form doesn’t work

Hi everyone,

I am trying setting up this calendar for appointment bookings.
I insert it with Elementar into an accordion.

But when I try to set an appointment I do not receive a confirmation. Neither as the host as no customer.

Why is the submit button not working?

Anyone an idea?
