The cause of the problem is most probably a conflict with another plugin.
Could you please, try to deactivate all other plugins, except Form maker and see whether they will appear?
If the problem will not be occurring, please activate plugins one by one and check Form maker simultaneously, to find which one is causing the issue.

Thanks for you answer. I tried it but didn’t work out.
I have added a ‘blank’ form to the same page and now it looks okay, but no the submit button isn’t working in IE and Firefox. In Chrome everything works fine.
I think it has something to do with the table. I’ll try something with another table…
I changed everytime with no tables. It seems to work fine now.
